*CRB\/DBS<\/strong>
Current CRB or update service reference number<\/p>

*Proof of Address<\/strong>
2 recent utility bills issued within 3 months<\/p>

*National Insuarance number<\/strong>
NI card, P45\/60 or payslip<\/p>

*Passport Photos<\/strong>
2 passport sized photos for your ID badge<\/p>

*Mandatory Training<\/strong>
Moving and Handling, Basic life support conducted face to face and others as listed on the training subsection.<\/p>

*Qualifications<\/strong>
Certificates and proof of registration (nurses, physiotherapists, occupational therapists etc)<\/p>

*Immunisation<\/strong>
Evidence of MMR, Hep B, TB and Vericella immunity\/ recent fitness to work certificate( for hospital assignments)<\/p>

*Reference Details<\/strong>
2 referees include their name, contact number, email.
References should be given by a manager or a senior and should cover a period of 3years.
Work emails only, personal email accounts are not accepted.<\/p>
