Passport, drivers licence
*CRB/DBS
Current CRB or update service reference number
*Proof of Address
2 recent utility bills issued within 3 months
*National Insuarance number
NI card, P45/60 or payslip
*Passport Photos
2 passport sized photos for your ID badge
*Mandatory Training
Moving and Handling, Basic life support conducted face to face and others as listed on the training subsection.
*Qualifications
Certificates and proof of registration (nurses, physiotherapists, occupational therapists etc)
*Immunisation
Evidence of MMR, Hep B, TB and Vericella immunity/ recent fitness to work certificate( for hospital assignments)
*Reference Details
2 referees include their name, contact number, email.
References should be given by a manager or a senior and should cover a period of 3years.
